If signing up for EZPay, I authorize my financial institution to charge my bank account or credit card monthly by the amount selected on the front of this form and to make that payment to Newspaper Agency Co., LLC (NAC). The monthly charge will appear on my statement for the date (or first business day thereafter) that I circled. I understand that I will be notified at least 10 days prior to the payment date if the amount is different. In addition, I have the right to stop payment of a charge by notifying NAC up to three business days prior to the charging of my account. This authority will remain in effect until revoked by me, my financial institution or NAC. I will notify NAC of any address, financial institution or account changes. If signing up for a new subscription, my household has not subscribed to The Salt Lake Tribune or Deseret News during the past 30 days. Any balance owed from prior subscriptions will be deducted from monies received on this subscription.

Payment information: When you make a payment by mailing a check to NAC, you are authorizing NAC to collect that check electronically. NAC will send the information on your check to your bank electronically as soon as the same day NAC receives your check. Your bank will deduct the amount of your check and show the transaction on your bank statement with NAC identified as payee of a one time electronic debit. If the payment is dishonored, NAC may re-present the payment electronically. NAC does not send the original check to your bank, and NAC will destroy the original check and retain only an image of the check. If your payment is returned after re-presentment, NAC may assess a return payment fee of $20.00. If you choose to opt-out of electronic presentment of your checks, you must contact NAC in a letter (separate from your payment). The publisher reserves the right to change subscription rates during the term of any subscription. Rate changes may be implemented by shortening the duration of the subscription term.

All home delivery subscriptions will include the Thanksgiving Day edition which will be charged to the subscriber's account at the Sunday rate. To opt out of the Thanksgiving Day edition or any other bonus delivery days (listed above) please contact customer service at 801-204-6100. If you do not opt out, the cost of your subscription will not change, but the term may be shortened.
